Claims Procedure

 

Upon the occurrence of any event giving rise to or likely to give rise to, under the certificates, the insured shall:

 
  • Give immediate notice in writing via email or via telephone or by letter to OUIC stating the circumstances of the loss. No claim shall be deemed valid unless notice of loss is duly received by OUIC.
  •  Inform the police immediately and take all steps within your power to minimize the extent of loss or damage or protect the affected property from further deterioration.
  •  OUIC shall, at its discretion, determine whether a Loss Adjuster/ Surveyor are required to assess the loss or otherwise upon receipt of the loss notification.
  •  Preserve all parts affected and make them available for inspection by a representative or surveyor/adjuster appointed by OUIC.
  •  Furnish to OUIC all such particulars and documentary evidence to substantiate the claim. The Insured shall bear the expenses of all such particular of documentations to substantiate the claim.
  •  The Insured shall advise whether the interest and/or property were insured with other insurers and give details of such insurers.
  •  The Insured are not allowed to negotiate or agree to on payments or on repairs or to commit in any manner to any Third Party without the prior consent of OUIC.
 

Fire

Claims Procedure


  • Claim form, dully filled in & signed by the Insured.
  • Inventory of loss.
  • Repairs bills.
  • Laboratory test Reports, if required.
  • Photographs, if taken by the Insured.
  • Departmental notice about the incident.
  • Fire brigade Report.
  • Police & Civil Defence Reports.

 

Engineering Claims

Claims Procedure


The documents listed below are applicable generally across all the policies under this branch. Extra documents specific to the type of policy are mentioned separately in respective sections:


  • Copy of the policy & Endorsement (if any).
  • Survey report.
  • Claim form duly filled in and signed.
  • Estimate of loss
  • Departmental note regarding the cause and nature of loss along with supporting log books if any.
  • F.I.R if necessary. (In case of malicious act; involvement of TPPI, TPPD).
  • Brigade, Civil Defence and Police Report (in case of loss due to fire).


A) Machinery Insurance


  • As mentioned in the list of suggested documents.


B) MB loss of Profit Policy:


  • Claim form duly filled in and signed by the Insured.
  • Books of Accounts.
  • Business Trends, Production & sales record.
  • Details about the savings in standing charges.
  • Details of increased cost of working.


    C) Boiler & Pressure Plant Explosion:


    A part from the documents suggested in the general list following documents will be called:


  • Latest Boiler inspector’s report.
  • Boiler inspection report on the incident.

    D) Electronic Equipment Policy:


  • As mentioned in the list of Suggested Documents. In addition to this copy of A.M.C, if any, on case to case basis.

    E) Erection All Risk Policy:


  • As mentioned in the list of Suggested Documents. Further additional documents may be called for depending on the nature of the claim.

    F) Contractors All risk Insurance:


  • As mentioned in the list of Suggested Documents. Further additional documents may be called for depending on the nature of the claim.

    G) Contractors Plant & Machinery Policy:


  • As mentioned in the list of Suggested Documents. Further additional documents may be called for depending on the nature of the claim.

 

Liability Loss

  • Depending on individual circumstances, following documents/ Information may be called/ verified.
  • Detailed version about the incident/ alleged misfeasance.
  • Details of loss caused/ injury/ death/ including all available information on victims as well as estimated quantum of liability.
  • Steps taken by the Insured to mitigate the loss.
  • Statements from witnesses, photographs, visual records of evidence/ circumstance, video, etc.
  • Any other evidence in support of claim.
  • Press reports.
  • FIR/ investigation report of Police.
  • Survey/ Investigation Report; Survey Report on the damaged property.
  • All notices/ summons of court.
  • Post Mortem Report/Medical Certificate, wherever applicable.
  • Weather (Meteorological) Report, wherever applicable.
  • Pollution Control Board Report, wherever applicable.
  • Consumer Action Group/ Society? Group presentation/ Report, wherever applicable.
 

Workmen's Compenstion

  • Claim form duly completed and signed.
  • ID or labour card copy.
  • Medical bills.
  • Doctor prescription.
  • Medical Reports.
  • Discharge summary.
  • Sick leave certificate(s).
  • Disability certificate in case of Permanent Total Disability.
  • Death certificate in case of death.
  • Passport copy.
  • Police report if required.
 

Personal Accident

  • Claim form duly completed and signed
  • ID or Labour card copy.
  • Medical bills
  • Doctor prescription.
  • Medical Reports.
  • Discharge summary.
  • Sick leave certificate(s).
  • Disability certificate in case of Permanent Total Disability.
  • Death certificate in case of death.
  • Passport copy.
  • Police report if required.
 

Travel

Details & Documents for Travel Insurance Claim


  • Policy number
  • Name of insured person
  • Your current contact number
  • Permanent contact number & email id (mandate)
  • Diagnosis in case of medical cases
  • Place and country of loss
  • Provider name
  • Contact details and the name of the caller (in case the person intimating the claim is not the insured)

Loss of Baggage

Delay of Baggage


  • Claim Form

  • Property Irregularity Report (obtained from Airline)
  • Original bills/receipts/invoices pertaining to expenses incurred/purchases made during the delay period.


Trip Delay/ Cancellation/ interruption/ missed connection


  • Confirmation from the airlines, clearly mentioning the scheduled arrival time and the actual arrival time
  • Copies of correspondence with the airline authorities certifying the delay
  • Medical reports and doctors statement,if trip is cancelled or interrupted due to medical reasons
  • Termination letter from the company,if trip is cancelled or interrupted due to employment reason
  • Original tickets of the insured and the traveling companion indicating travel to the same destination for the same dates,in case the cancellation or interruption is owing to the sickness, injury or death of a traveling companion
  • All the bills / receipts of reasonable additional expenses incurred and / or proof of cancellation charges levied by the carriers


Loss of Passport


  • Copy of new passport
  • Copy of previous passport (if available)
  • Original bills/invoices of expenses incurred for obtaining a new passport
  • Copy of FIR/ Police Report


Personal Liability


  • Full statement of the facts in writing
  • Witness statements
  • Any other documents relevant to the incident, including Summons, legal notice etc.
